Please follow the instructions and complete ALL required steps in order to be considered for the 2025 HAWAI’I SWIM SHOW MODEL CAST.
STEP ONE:
Add to Cart & Proceed with service fee payment.
STEP TWO:
Complete and submit the attached “Model Application Form”.
STEP THREE: Submit Model Comp Card to:

with subject- FIRST NAME & LAST NAME 2025

***Please note: There is a $25 non-refundable service fee for the processing of all kāne & wāhine applications. $5 for keiki applicants 15 years old and under***
